Registering at the Practice

Patients wishing to register at the practice need to either bring their medical cards into the surgery or collect a Registration Form from the surgery for completion.  For each patient over the age of 5 years a new patient questionnaire also needs to be completed at the time of registration.
All patients 16yrs and over will be asked to attend for a new patient health check with our Health Care Assistant.
You will also receive a Care Data pack, this includes comprehensive information to enable you to make an informed decision on data sharing.  Your decision will be added to your medical records.

Area covered by the practice

The practice lies within the Newcastle Gateshead CCG area of Rowlands Gill.

This is the area covered by the NE39 post code up to Hamsterley Mill.  Although Hamsterley Mill has an NE39 postcode it lies within the County Durham and Darlington Health Authority area and we are unable to accept patients living there.  We do not accept patients from the NE17 or NE21 areas although we currently have a few patients from Chopwell and Winlaton Mill who have been registered with us for many years and were allowed to remain on our list when the practice boundaries changed.
